How it works
1. Fill in the form below. Tell us who you are and which GP surgery you are registered with.
2. We confirm your details. Our team checks everything and sets Warmsworth Road Pharmacy as your nominated pharmacy on the NHS system. We will contact you if we need anything else.
3. Your prescriptions arrive automatically. From then on, every prescription your GP issues comes straight to us. We will send you a text message when it is ready to collect – and if you are housebound or vulnerable, we can deliver to your door free of charge.

The bits people always ask
Do I need to tell my GP?
No. Once you have completed this form, we set your nomination on the NHS Electronic Prescription Service on your behalf. Your GP practice sees this automatically.
Does it cost anything?
No. Nominating a pharmacy is a free NHS service. Your usual prescription charges or exemptions still apply.
Can I still order repeat prescriptions?
Yes. You can order repeats through your GP surgery, the NHS App, or ask us about managing your repeat medication for you.
Do you deliver?
Yes. We deliver free of charge to housebound and vulnerable patients. Just mention it in the comments when you nominate us, or give us a call.
What if I move or want to change pharmacy?
You can change your nomination at any time – just let us, your GP or any other pharmacy know.
